Job Description

Job Summary:

  • The Clinical Dietitian will provide nutrition screening, assessment, and follow-up care for patients based on level of care.
  • This role involves evaluating patient nutritional needs, providing counseling and education, and collaborating with healthcare teams to ensure effective dietary plans.
  • The Dietitian will work closely with patients and their families to incorporate food preferences, manage food/drug interactions, and support improved patient outcomes.

Responsibilities:

  • Screen and assess patients to determine nutritional status and needs.
  • Conduct interviews with patients and families regarding dietary history and preferences.
  • Monitor food-drug interactions and provide relevant education.
  • Interpret clinical and laboratory data for nutrition-related decision-making.
  • Document diet instructions, educational sessions, and patient understanding.
  • Collaborate with physicians on appropriate diet modifications.
  • Adjust and personalize patient menus as needed.

Education and Experience: 

  • Bachelor’s degree in Dietetics or Foods and Nutrition from an accredited institution.
  • Minimum 1 year (2 years preferred) in clinical nutrition.

Certification (Acute Care):

  • Registered Dietitian (RD) with Commission on Dietetic Registration (CDR) – Required.

Certification (Post-Acute Care): 

  • NYS Certification as “Dietitian-Nutritionist” by NYSED – Required at time of hire.
  • Preferred: AHA Basic Life Support (BLS) certification.
  • Preferred: Master’s Degree in Nutrition or related field.
  • Primary Source Verification: Required.

Additional Requirements:

  • Must meet federal long-term care facility compliance standards.
  • Must demonstrate strong documentation, communication, and collaborative skills.
