Job Summary:
- The Clinical Dietitian will provide nutrition screening, assessment, and follow-up care for patients based on level of care.
- This role involves evaluating patient nutritional needs, providing counseling and education, and collaborating with healthcare teams to ensure effective dietary plans.
- The Dietitian will work closely with patients and their families to incorporate food preferences, manage food/drug interactions, and support improved patient outcomes.
Responsibilities:
- Screen and assess patients to determine nutritional status and needs.
- Conduct interviews with patients and families regarding dietary history and preferences.
- Monitor food-drug interactions and provide relevant education.
- Interpret clinical and laboratory data for nutrition-related decision-making.
- Document diet instructions, educational sessions, and patient understanding.
- Collaborate with physicians on appropriate diet modifications.
- Adjust and personalize patient menus as needed.
Education and Experience:
- Bachelor’s degree in Dietetics or Foods and Nutrition from an accredited institution.
- Minimum 1 year (2 years preferred) in clinical nutrition.
Certification (Acute Care):
- Registered Dietitian (RD) with Commission on Dietetic Registration (CDR) – Required.
Certification (Post-Acute Care):
- NYS Certification as “Dietitian-Nutritionist” by NYSED – Required at time of hire.
- Preferred: AHA Basic Life Support (BLS) certification.
- Preferred: Master’s Degree in Nutrition or related field.
- Primary Source Verification: Required.
Additional Requirements:
- Must meet federal long-term care facility compliance standards.
- Must demonstrate strong documentation, communication, and collaborative skills.
