Clinical Pharmacist

Job Summary:

• We are seeking an experienced Clinical Pharmacist to join our MTM product team.
• The pharmacist will provide medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMRs) and in-depth consultations on statin medication use per AHA guidelines.
• The role involves transcribing patient-specific prescription and diagnosis information into a secure, third-party web application. This data will generate standardized documents mailed to patients.

Duties and Responsibilities:

• Conduct up to 100 outbound patient calls per day.
• Perform Comprehensive Medication Reviews (CMRs).
• Deliver MTM services with accuracy and professionalism.
• Enter prescription and diagnosis data into a third-party web-based application quickly and accurately.

Knowledge & Experience:

• Minimum 4 years as a Clinical MTM Pharmacist (management experience preferred).
• Experience driving Part D Star Measures (SUPD, SPC, and CMR).
• Strong knowledge of pharmaceutical and medical vocabulary.
• Proven clinical experience in medication review.
