Job Summary:
- Responsible for delivering high-quality pharmaceutical care by managing medication therapy, preventing potential drug-related issues, and resolving medication-related problems.
- All responsibilities are performed in a patient-centered manner across all age groups (pediatrics to adults).
Key Responsibilities:
- Review and verify medication orders for accuracy, dosage, allergies, and drug interactions.
- Ensure safe and appropriate medication dispensing with minimal errors.
- Accurately enter and process medication orders in the system.
- Deliver medications within established timelines and guidelines.
- Monitor patient therapy and recommend appropriate interventions.
- Maintain compliance with hospital policies, safety standards, and infection control practices.
- Prepare sterile products using proper aseptic techniques.
- Ensure adherence to clean room protocols and sterile compounding standards.
- Collaborate with physicians, nurses, and healthcare teams to optimize patient care.
Requirements:
- Bachelor’s Degree or PharmD in Pharmacy required.
- Active RPH license in the state of Texas required.
- Minimum 2 years of hospital pharmacist experience (Pediatric experience preferred).
- Completion of 40 hours of sterile compounding training (ACPE or ASHP accredited).
- Clean room experience preferred.
