Job Description

Job Summary:

  • Under general supervision, the Social Worker II provides professional social services to patients and their families.
  • This includes psychosocial assessments, crisis interventions, discharge planning, and advocacy to support optimal patient care and outcomes.

Duties and Responsibilities:

  • Provide social services to patients and families within an assigned medical service area.
  • Conduct discharge planning evaluations and psychosocial assessments.
  • Communicate findings and concerns to the healthcare team.
  • Offer counseling related to disease, treatment, financial or social challenges, and emotional stress.
  • Identify high-risk social cases and provide crisis intervention (e.g., child abuse/neglect, physical abuse).
  • Serve as a liaison between patients/families, hospital staff, and external agencies.
  • Evaluate and adjust social work services to achieve desired patient outcomes.
  • Assist with discharge planning, placement, and referrals to post-acute or community-based services.
  • Support patients and families in managing difficult diagnoses, treatment transitions, or functional changes.
  • Advocate for patient rights, informed decision-making, and ethical care practices.
  • May collect data related to referrals, extended stays, or service utilization.
  • May supervise students or new employees as assigned.

Required Education:

  • Master’s Degree in Social Work (MSW) from an accredited school.

Preferred Licensure/Certification:

  • Licensed Master Social Worker (LMSW) strongly preferred.

Experience:

  • Hospital or healthcare setting experience preferred.

Knowledge, Skills, and Abilities:

  • Strong knowledge of social work principles, casework practices, and community resources.
  • Familiarity with current social, economic, and healthcare trends and issues.
  • Excellent analytical and problem-solving skills.
  • Ability to communicate technical or sensitive information clearly to patients, families, and medical staff.
  • Strong interpersonal and communication skills.
  • Specialized interviewing skills, including working with children.
