Job Description
Duties and Responsibilities:
- Assess patients and families referred to Social Work services in a timely manner.
- Integrate social work plans into overall patient care through interdisciplinary collaboration.
- Provide skilled services including crisis intervention, case management, discharge planning, advocacy, and patient/family education.
- Manage psychosocial issues such as abuse/neglect, advanced directives, psychiatric placement, and end-of-life concerns.
- Utilize hospital and community resources to ensure continuity and quality of care.
- Refer patients, families, and staff to appropriate services; assist with insurance coverage for services.
- Document care plans, interventions, and outcomes accurately in the medical record.
- Complete statistical reports as required.
- Collaborate with interdisciplinary teams on complex discharge and transitional care plans.
- Participate in professional development, community education, and support group activities.
- Exercise independent judgment when making decisions with minimal review.
- Work directly with patients, families, hospital staff, internal and external customers, and healthcare leadership.
Requirements:
- Master of Social Work (MSW) or closely related degree from an accredited program.
- Prior experience working with the assigned patient population.
- Hospital or community health agency experience.
- Hospice experience (hospital, community, and in-office).
- Computer literacy.
- Additional language fluency.
- LCSW (Licensed Clinical Social Worker) or LPC (Licensed Practical Counselor) preferred.
