Job Description

Duties and Responsibilities:

  • Assess patients and families referred to Social Work services in a timely manner.
  • Integrate social work plans into overall patient care through interdisciplinary collaboration.
  • Provide skilled services including crisis intervention, case management, discharge planning, advocacy, and patient/family education.
  • Manage psychosocial issues such as abuse/neglect, advanced directives, psychiatric placement, and end-of-life concerns.
  • Utilize hospital and community resources to ensure continuity and quality of care.
  • Refer patients, families, and staff to appropriate services; assist with insurance coverage for services.
  • Document care plans, interventions, and outcomes accurately in the medical record.
  • Complete statistical reports as required.
  • Collaborate with interdisciplinary teams on complex discharge and transitional care plans.
  • Participate in professional development, community education, and support group activities.
  • Exercise independent judgment when making decisions with minimal review.
  • Work directly with patients, families, hospital staff, internal and external customers, and healthcare leadership.

Requirements:

  • Master of Social Work (MSW) or closely related degree from an accredited program.
  • Prior experience working with the assigned patient population.
  • Hospital or community health agency experience.
  • Hospice experience (hospital, community, and in-office).
  • Computer literacy.
  • Additional language fluency.
  • LCSW (Licensed Clinical Social Worker) or LPC (Licensed Practical Counselor) preferred.
