Job Description
Duties and Responsibilities:
- Initiate MD orders for all MSW-PCLS consults.
- Conduct initial psychosocial assessments for patients on legal hold using the EPIC SW Navigator system.
- Complete the Social Determinants of Health (SDOH) section for every admitted patient.
Collaborate with psychiatric providers to determine whether cases require full legal hold monitoring or are “consult only.” - Perform daily rounding for patients on active legal holds; see LPS conserved patients three times weekly.
- Work to transfer patients to Riverside County LPS-approved facilities or Intensive Treatment Facilities (ITFs) when medically cleared.
- Monitor transitions between legal statuses (e.g., 5150 to 5250 to TCON) and ensure proper documentation and advisement.
- Assess patients who are gravely disabled (GD) for timely LPS conservatorship referrals during the first week of admission.
- Maintain strong communication with nursing leadership, case managers, community agencies, and patient families.
Coordinate discharge planning, including referrals to:
- Mental health services.
- Substance use disorder programs.
- Housing and shelter resources.
- Transportation and non-medical placement.
Education, Experience and Certifications/ License:
- Must have either an interim BBS license (AMFT/ACSW/APCC).
- Master’s degree in social work required (licensed not required)
- Minimum two year’s of experience in clinical setting required, will assist with our current SW staff shortage.
- Previous counseling experience preferred.
- A worker with mental health crises intervention experience.

- Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position; operate/troubleshoot basic office equipment; type 30 words per minute without errors.

- Able to distinguish colors as necessary; write, hear, read and speak sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with workplace; see adequately to read computer screens and written documents necessary to the position.
