Job Description
Duties and Responsibilities:
- Along with multi-disciplinary team, engages 68 clients in community and office visits at least once weekly in order to provide ongoing wellness and medication management, assessment, and other supports as needed.
- Provides direct services related to the treatment and recovery of people living with mental illness according to ACT guidelines.
- Demonstrates competencies in harm reduction, trauma-informed care, stage-wise treatment, motivational interviewing, and other evidence-based practices.
- Displays core competencies in designated areas of practice, including the ACT core operating principles, integrated physical health and behavioral health, vocational services, family psychoeducation, and wellness self-management.
- Coordinates treatment and services with a variety of other teams and systems including inpatient units, primary and specialty care physicians, supportive housing programs, substance use rehabilitation programs, respite care, etc.
- Completes goal-oriented treatment planning (plan of care, psychosocial, safety plans, etc.)
Required Education:
- Master’s Degree in Social Work.
Required Certifications & Licensure:
- Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification.
- SEL/SCR certification is required.
Required Skills & Experience:
- One (1) year of LMSW experience.
