Job Description
Duties and Responsibilities:
- Conduct mental health screening assessment interviews utilizing symptom measures, documenting assessments in Epic and databases, and presenting cases at weekly team meetings.
- Provide psychosocial assessments.
- Provide benefits counseling.
- Develop appropriate disposition plans for each patient and identify/provide appropriate mental health and social service referrals.
- Participate as part of a multidisciplinary team, including both mental health and medical providers, to coordinate patient care.
- Work closely with related social and community agencies.
- Provide professional and appropriate interventions during crisis or emergency situations.
- Complete appropriate documentation in the electronic medical record.
- Manage an estimated workload of 25–30 assessments per week, including initial and monitoring patients, along with up to 10 weekly mental health/social service/external provider referrals.
Education:
- Master’s Degree in Social Work.
Required Certifications & Licensure:
- Valid Licensed Master Social Worker (LMSW) license issued by NYSED, Primary Source Verification.
Required Skills & Experience:
- One (1) year of LMSW experience.
