Job Description

Duties and Responsibilities:

• Conduct psychosocial assessments.
• Provide supportive therapy.
• Coordinate discharge planning.
• Make referrals for appropriate services.
• Support patients within an inpatient medicine/surgery setting.

Required Education:

• Master’s Degree in Social Work.

Required Certifications & Licensure:

• Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification.

Required Skills & Experience:

• Minimum 1 year of LMSW experience.
• Psychosocial Assessment, Supportive Therapy, Discharge Planning, Referral Management, Inpatient Medicine/Surgery Experience.
