Job Description
Duties and Responsibilities:
- Evaluates and treats all referred Physical Therapy patients according to physician orders.
- Performs and supervises physical therapy programs in the homes of client patients.
- Conducts initial evaluations, establishes treatment plans with patients/caregivers, and regularly updates goals.
- Administers treatment based on current orders and supervises PTAs or HHAs as appropriate.
- Tracks and documents functional improvement supported by objective changes.
- Completes documentation according to department standards, reflecting skilled, necessary interventions.
- Maintains productivity per department expectations and uses time efficiently.
- In Client health, adheres to OASIS accuracy, safety standards, and responsible resource utilization.
- Participates in quality improvement initiatives and regulatory compliance.
Education:
- Bachelor’s, Master’s, or Doctorate degree from an accredited Physical Therapy program (Required).
Licensure & Certifications:
- Wisconsin state license as a Physical Therapist (Required).
- AHA Basic Life Support for the Healthcare Provider (BLS) or AHA-approved equivalent (Required).
Required Skills & Experience:
- Good communication skills.
