Job Description
Job Summary:
- Responsible for developing, implementing, and evaluating individualized nutrition care plans for patients across the lifespan.
- Provides comprehensive nutrition assessments, education, and ongoing reassessment to ensure optimal patient outcomes, with a focus on medical nutrition therapy and diabetes management.
Key Responsibilities:
- Develops, implements, and evaluates patient-specific nutritional care plans.
- Provides age-appropriate nutrition assessments for patients identified at nutrition risk, including diabetes management and education within established guidelines and timeframes.
- Adapts nutrition plans to meet patients’ economic, social, cultural, and age-specific needs.
- Reassesses patients at appropriate intervals and adjusts the plan of care as needed.
- Delivers age-appropriate nutrition education to patients and families.
- Assesses learning needs and readiness to learn for patients and caregivers.
- Participates in the development and review of educational materials and departmental resources to meet population needs.
Required Knowledge & Skills:
- Strong knowledge of nutrition principles and medical nutrition therapy.
- Pediatric nutrition knowledge preferred/required as applicable.
- Ability to identify nutritional deficiencies and develop corrective action plans.
- Skilled in conducting assessments and making evidence-based recommendations.
- Ability to educate patients, families, and healthcare staff on nutritional needs.
- Proficient in computer applications including word processing, spreadsheets, and nutrition software programs.
