Job Description

Job Summary:

• As a member of an interdisciplinary team, provides quality physical therapy services through comprehensive evaluation, program planning, and administration of treatment to all patients.
• Performs detailed clinical assessments based on individual patient needs upon physician referral and will develop comprehensive age-appropriate plans of care based on findings following physical therapy evaluation.
• Accurately and appropriately documents patient treatment.
• Responding effectively to new and changing work assignments is a must.

Education and Certifications:

• Degree in Physical Therapy – Required.
• BLS – Required.
• First Aid certification preferred.
• PT License – Required.
• State Licensure Required.
• Primary Source On-Line Verification – Required.

Required Experience:

• Must have inpatient rehab experience.
• Physical Therapy Experience – Required: 1 Year inpatient rehab or acute experience.
• Crisis Prevention training (TEAM) required within 6 months of hire (PRN-N/A).

Additional Requirement:

• Must live within 100 miles.
