Job Description
Job Summary:
- As a member of an interdisciplinary team, provides quality physical therapy services through comprehensive evaluation, program planning, and administration of treatment to all patients.
- Performs detailed clinical assessments based on individual patient needs upon physician referral and will develop comprehensive age-appropriate plans of care based on findings following physical therapy evaluation.
- Accurately and appropriately documents patient treatment.
- Responding effectively to new and changing work assignments is a must.
Education and Certifications:
- Degree in Physical Therapy – Required.
- BLS – Required.
- First Aid certification preferred.
- PT License – Required.
- State Licensure Required.
- Primary Source On-Line Verification – Required.
Required Experience:
- Must have inpatient rehab experience.
- Physical Therapy Experience – Required: 1 Year inpatient rehab or acute experience.
- Crisis Prevention training (TEAM) required within 6 months of hire (PRN-N/A).
Additional Requirement:
- Must live within 100 miles.
