Job Description

Job Summary:

• We are seeking a Licensed Clinical Social Worker (LCSW) with strong experience in behavioral health and hospital case management.
• This short-term assignment is ideal for a professional who thrives in dynamic environments and has a solid understanding of post-discharge planning and community resources.

Duties and Responsibilities:

• Provide psychosocial support and discharge planning for patients across various inpatient settings.
• Assess patients’ social, emotional, and financial needs impacting care and recovery.
• Coordinate care and referrals to community resources, facilities, and support services.
• Maintain accurate and timely documentation per hospital policies.
• Collaborate with a multidisciplinary team to ensure continuity of care.

License:

• Active LCSW license through the Nevada State Board of Examiners for Social Workers – Required.

Experience:

• Minimum 1 year of experience in a hospital setting – Required.
• Familiarity with discharge planning and post-acute care resources.

Knowledge, Skills and Abilities:

• Excellent communication and assessment skills.
• Knowledge of community-based services and resources.
• Ability to work independently and manage a dynamic caseload.
• Must demonstrate age-appropriate care/service competencies for the assigned population.
