Job Description
Performs Patient assessments:
- Accurately assesses patient’s appropriateness for home care, learning needs and progress toward goals.
- Collaborates with patient, family, and physician to develop the plan of care for the patient.
- Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.
- Maintains safe environment for patients and co-workers:
- Assesses social environment and adjust or provide education as needed.
- Assess for potential clinical risks and adapt program as indicated.
- Follows universal infection control measures.
Implements treatment program to achieve designated goals:
- Administer occupational therapy services per physician orders.
- Instruct in communication and safe swallow, cognitive training as indicated.
- Develop and instruct in therapeutic exercise program.
- Implement program to restore function, and achieve maximum functional status.
- Provides discharge planning and referral to resources as needed.
- Communicate with physician, nurse, and other clinical disciplines as necessary.
- Develops/implements/evaluates plan of care based on evidence-based knowledge.
- Revise care plan as indicated within physician orders.
Uses age appropriate teaching methods for patients and families:
- Evaluates teaching plan and revises as necessary.
- Documents patient education tools utilized.
Demonstrates compliance with agency policies:
- Assumes responsibility for Case Management.
- Demonstrates knowledge of home care regulations and insurance reimbursement guidelines.
- Assures compliance with regulatory requirements.
- Follows agency documentation policies.
- Shows evidence of interdisciplinary communication.
- Ensures appropriate utilization of services.
- Follows scheduling process.
- Maintains visit productivity standards.
- Participates in Team and IDT meetings.
- Prioritizes schedule to accommodate patient’s needs.
- Travels to patient homes and facilities as indicated.
- Complies with paid time off request process and weekend/holiday requirements.
Assists with growing agency business:
- Provides orientation to new employees.
- Accepts other assignments as requested.
- Cross trains to learn new skills.
- Use of personal vehicle is required for home visits.
Required Education:
- Graduate of an accredited speech and language program with a minimum of a Master’s degree.
Required Certifications & Licensure:
- Current NJ ST License.
Required Skills & Experience:
- Minimum of one (1) year of experience in acute, subacute or home care setting.
- Good customer service and communication skills.
