Master Social Worker

Job Description

Duties and Responsiblities:

• Conduct psychosocial assessments and identify patients’ social, emotional, and financial needs.
• Provide counseling, emotional support, and crisis intervention to patients and families.
• Coordinate discharge planning, referrals, and continuity of care after hospitalization.
• Liaise with doctors, nurses, and multidisciplinary teams to address patient needs.

Requirements:

• Master’s Degree in Social Work from an accredited school of social work required.
• Must have hospital experience working in inpatient adult discharge planning.
• Must have active/valid NJ license.

Note: Non-Taxable per diem is provided to the qualified candidate living more than 50 miles from the facility.
