Job Description

Job Summary:

  • Provides direct and indirect family centered therapy services for patients to increase functional abilities, independence and community integration.
  • Contributes to department, program and service teams to assure achievement of desired outcomes.
  • Specific duties are determined by the work setting or patient population served; however, in all positions, serving patients with challenging and complex conditions requires a concentration of specialized staff able to work in highly coordinated teams.
  • We organize care around each patient’s condition which requires staff at multiple levels and across many departments to exhibit exceptional teamwork qualities including communicating efficiently and effectively and adopting the behaviors required for high functioning integrated teams, consistent with our Product CARES expectations.

Duties and Responsibilities:

  • Completes an evaluation utilizing principles of family-centered care.
  • Designs assessment consistent with goals of evaluation, provider order, patient / family input and priorities.
  • Obtains necessary history and background information.
  • Selects evaluation methods, tools, and environment to obtain valid and reliable assessment specific to the patient’s age and developmental, neurological, musculoskeletal, cardiopulmonary, cognitive and behavioral status.
  • Administers evaluation consistent with best practices and Product competencies.
  • Interprets evaluation for to the patient / family and seeks their assessment of the evaluation’s reliability and validity.
  • Makes recommendations for treatment, referrals, equipment, resources and home and community based activities.
  • Documents and bills for evaluation based on education and guidelines provided by the Product Children’s Specialty Healthcare Rehabilitation Therapies Revenue Cycle Team.
  • Excellent customer service skills.

Licensure and Certification:

  • Physical Therapy State License o Pending credentials:
  • Registration and Licensure pending, not to exceed three months in this status for PT.
  • Current certification in CPR (BLS for Healthcare Providers through American Heart Association) or must obtain certification within 90-days of hire.

Knowledge, skills, abilities:

  • Knowledge of child growth and development, rehabilitation theory and practice with a wide variety of childhood onset diagnoses and related considerations including anatomy and neurology, emotional behavior factors, family needs and issues, community transition and equipment.
  • Effective verbal and written communication.
  • Ability to function as an effective team member.
  • Competency in physical therapy evaluation, treatment and related family/community education and planning for transition to community services.

Preferred Knowledge, skills, Abilities:

  • transition from pediatric to adult health and rehabilitation care and of rehabilitation practice for adults with Childhood onset disabilities.
  • Requires specialty knowledge in human growth and development and multiple rehabilitation and habilitation program and service areas.
  • Effective team functioning requires working with a wide variety of disciplines at Product Children’s Specialty Healthcare, the patient and family, community education and medical professionals, representative of public and private providers, interpreters, advocates and payers.
  • Providing on-going comprehensive education, guidance and support for patient, family, and community.
