Requirements:

  • Long term care experience is critical.
  • The position does involve discharge planning, assessments, and working with behaviour medicine group to meet patient needs as well as regulations.
  • Need someone who has worked in a nursing home under current regulations.
  • Skilled nursing facility experience needed MI License is a must have 8D with weekend/holiday rotation.
