Anticoagulation Pharmacist

Job Description

Job Summary:

• Responsible for providing comprehensive management of patients receiving anticoagulant therapy in the outpatient setting.
• The pharmacist collaborates with physicians, nurses, and other healthcare professionals to ensure safe, effective, and evidence-based anticoagulation care.
• This includes patient education, laboratory monitoring, dosage adjustments, documentation, and coordination of care in accordance with established clinical protocols and national guidelines.

Required Skills & Experience:

• Experience with Anticoagulation.
• Preferred Skills & Experience:Anticoagulation management experience.
• Experience with the Product Program.
• Required Certifications& Licensure:Must be a licensed pharmacist in the State of Massachusetts.
