Speech Pathologist

Job Description

Duties and Responsibilities:

• Provide service to patients who have speech and language problems.
• Participate in the development of speech therapy plans of care for patients.
• Make visits to the patients for initial and ongoing assessments to identify the patient’s level of functioning and ability to communicate, including OASIS Assessments at appropriate time points.
• Communicates with the physician before start of care and as needed.
• Implement the plans of care.
• Provide education and instruction to team members, patients and families.
• Document care and submit paperwork in an accurate and timely manner to update and maintain the patient medical records.

Required Skills & Experience:

• 1 year of experience (preferably in a clinic setting) is required.

Required Certifications & Licensure:

• Current/valid state Speech Pathologist licensure is required.
• BLS – American Heart Association.
