Job Description

Responsibilities:

  • Review and optimize patient medication regimens to ensure safety, accuracy, and therapeutic effectiveness.
  • Identify and mitigate adverse drug events, therapeutic duplications, and medication interactions.
  • Ensure medication dosages are appropriate based on patient age, renal function, and liver function.
  • Participate in clinical rounds, interdisciplinary meetings, and patient care conferences.
  • Educate patients and families on medication usage when needed.
  • Provide drug information and in-service training to nursing, medical, and ancillary staff.
  • Support clinical protocols, pain management plans, and emergency code responses.
  • Maintain accurate medication records in compliance with local, state, and federal regulations.
  • Perform dispensing and other pharmacy duties as assigned.

Requirements:

  • Bachelor’s or Doctorate degree in Pharmacy (BS Pharm, MS, or PharmD preferred ).
  • Current and active California Pharmacist License ( Required ).
  • BLS Certification through the American Heart Association ( Required ).
  • Minimum of 1 years of inpatient hospital pharmacy experience ( Required ).
  • Experience with Meditech Electronic Health Record (EHR) system ( Required ).
