Job Description
Responsibilities:
- Review and optimize patient medication regimens to ensure safety, accuracy, and therapeutic effectiveness.
- Identify and mitigate adverse drug events, therapeutic duplications, and medication interactions.
- Ensure medication dosages are appropriate based on patient age, renal function, and liver function.
- Participate in clinical rounds, interdisciplinary meetings, and patient care conferences.
- Educate patients and families on medication usage when needed.
- Provide drug information and in-service training to nursing, medical, and ancillary staff.
- Support clinical protocols, pain management plans, and emergency code responses.
- Maintain accurate medication records in compliance with local, state, and federal regulations.
- Perform dispensing and other pharmacy duties as assigned.
Requirements:
- Bachelor’s or Doctorate degree in Pharmacy (BS Pharm, MS, or PharmD preferred ).
- Current and active California Pharmacist License ( Required ).
- BLS Certification through the American Heart Association ( Required ).
- Minimum of 1 years of inpatient hospital pharmacy experience ( Required ).
- Experience with Meditech Electronic Health Record (EHR) system ( Required ).
