Job Description
Job Summary:
- The Medical Social Worker provides psychosocial assessment, care planning, crisis intervention, and support to patients and families.
- The role works closely with interdisciplinary teams, caregivers, community resources, and CPS to address social, emotional, environmental, and discharge-related needs.
Duties & Responsibilities:
- Conduct comprehensive psychosocial assessments and reassessments.
- Develop, implement, and evaluate individualized plans of care.
- Provide psychosocial support, crisis intervention, and behavioral support.
- Coordinate with patients, families, caregivers, CPS, and interdisciplinary teams.
- Identify barriers to care and discharge and connect families with appropriate resources.
- Support families with complex dynamics, grief, bereavement, and end-of-life decision-making.
Conduct SI assessments/reassessments as required. - Assist with legally complex cases and appropriate community referrals.
- Maintain accurate documentation while protecting patient privacy and confidentiality.
- Participate in patient handoffs and collaborative family-centered care.
Required Qualifications:
- Active Texas LMSW or LCSW license, current and verifiable through the Texas BHEC.
- Experience with mental health populations preferred.
- Inpatient psychiatric, mental health authority, medical hospital, or CPS experience preferred.
- Strong assessment, critical thinking, communication, crisis intervention, and problem-solving skill.
