Job Description
Duties and Responsibilities:
- Screen and assess patients based on level of care and determine nutritional needs/status.
- Provide nutritional follow-up and ongoing patient care.
- Interview patients/families regarding food habits and preferences.
- Review and interpret laboratory, physical, and social information relevant to nutrition care.
- Provide nutrition education to patients/residents and families.
- Document diet instructions, education, and patient understanding in the medical record.
- Monitor and educate patients regarding food/drug interactions.
- Consult with physicians regarding appropriate diet modifications.
- Modify patient menus as needed.
- Provide nutritional care across different populations and disease states.
- Participate in performance improvement activities.
- Act as a member of the interdisciplinary care team.
Top 3 Non-Negotiable Skills:
- Registered Dietitian credential/registration.
- New York State Dietitian-Nutritionist certification.
- Clinical nutrition assessment and patient education experience.
Requirement:
- Minimum 1 year of experience required; 2 years preferred.
- Experience in Long Term Acute Care Hospital (LTACH) and/or Skilled Nursing Facility (SNF) environments.
- Experience with PointClickCare, SigmaCare, MDS 3.0, or similar electronic medical charting systems.
- Experience with electronic menu systems such as CBORD, NetMenu, or GeriMenu.
- Master’s degree preferred.
- AHA BLS preferred.
- Experience working with patients from diverse populations and disease states.
- Experience participating in interdisciplinary teams and performance improvement activities.
