Job Description

Job Summary:

• The PCLS Social Worker provides clinical social work services to patients under Psychiatric Consultation Liaison Services (PCLS), primarily those on legal holds (5150/5585, 5250, TCON, PCON).
• Responsible for conducting comprehensive psychosocial assessments, monitoring legal hold status, supporting conservatorship processes, and coordinating safe psychiatric discharge planning.
• Works collaboratively with Psychiatrists, Nursing, and Case Management to ensure continuity of care and appropriate community placement.

Duties:

• Manage PCLS consults & patients on legal holds (5150/5250/TCON/PCON).
• Complete EPIC assessments (including SDOH).
• Monitor legal hold status & conservatorship timelines.
• Coordinate psychiatric discharge planning & community placements.

Requirements:

• Master’s in social work (license not required).
• ACSW/APCC/AMFT Intern # or LCSW/LMFT/LPCC.
• 2+ years clinical experience.
• Mental health crisis intervention experience.
• Knowledge of Legal holds.
