Social Worker – LCSW

Job Description

Job Summary:

• Fulfills the needs of patients, families and community by providing Social Work services in accordance with his/her licensure, certification, training, and regulatory requirements.
• Assumes the responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit.
• Conducts discharge planning, coordination of care, and other planning and counseling activities to effectively move the patient through the continuum of care in a cost-effective and resource efficient manner.
• Responds effectively to new and changing work assignments.

Experience Required:

• At least 1 year of hospital case management (discharge planning) experience required.
• Knowledge of client.

Certifications Required:

• LICSW, active DC SW license.

Other Requirements:

• Must live 50 miles away.
